unnamedHerkimer County HealthNet is offering a Chronic Disease Self- Management class designed to help both people living with a chronic disease and also individuals caring for someone with a chronic disease. Chronic disease conditions include diabetes, heart disease, arthritis, chronic lung disease, asthma, fibromyalgia, and other long-term health conditions. This class will help participants learn to better manage their chronic conditions, and improve their quality of life.

The program will begin Monday, July 11and continue to meet ever Monday for six weeks from 2:30 to 5 p.m. at the Herkimer County Chamber of Commerce, 420 E. German St., Herkimer.

CDSMP workshops are designed to help people gain self-confidence in their ability to control their symptoms and learn how their health problems affect their lives. Small-group, highly interactive workshops are six weeks long, meeting once a week for two-and-a-half hours and are facilitated by two trained coaches.

Diabetes, arthritis, hypertension, lung disease, and other chronic diseases can make life difficult to manage for millions of adults, often forcing them to give up their independence. The program is especially helpful for these people, as it gives them the skills to coordinate all the things needed to manage their health, as well as to help them keep active in their lives.
