Community Resources for Independent Seniors is celebrating 10 years of serving the 55-plus community. We are proud to be able to offer another Five Wishes program that complements our Vital Conversations Series. We offer an intimate setting to walk you through every step of the Five Wishes booklet.

Five Wishes is changing the way we talk about advance care planning.

It’s more than just a document. Five Wishes is a complete approach to discussing and documenting your care and comfort choices. It’s about connecting families, communicating with healthcare providers and showing your community what it means to care for one another.

The Five Wishes Program will cover

  • Wish 1: The Person I Want to Make Care Decisions for Me When I Can’t
  • Wish 2: The Kind of Medical Treatment I Want or Don’t Want
  • Wish 3: How Comfortable I Want to Be
  • Wish 4: How I Want People to Treat Me
  • Wish 5: What I Want My Loved Ones to Know

This Five Wishes  program is special because we have a co-host from Hospice of CNY helping to lead the class. The workshop is 2 p.m. May 29, 2019.

A free Five Wishes booklet is available if you attend the program.

All are welcome to join us. If you have already completed a program or still need to fill in a few areas or you a just starting to think about it and just want to listen.

The CRIS mission is to identify and coordinate supportive services for individuals 55 and older living in the Cazenovia, Nelson and Fenner areas.
