State Police in Herkimer are investigating a three-vehicle fatal crash that occurred on State Route 20 in the Warren, Herkimer County.

On July 16, 2017, at approximately 12:15 p.m., a 1996 Dodge van was stopped on Route 20 attempting to make a left turn into the driveway at 4152 Route 20 when a 2003 Honda Motorcycle, towing a trailer, failed to observe the vehicle stopped and rear-ended the van. The operator of the motorcycle, Douglas A. Woolsey, and his passenger, Theresa A. Woolsey, were ejected from the motorcycle, which came to rest on its left side in the westbound lane. The force of the crash forced the van into the eastbound lane where it struck a 2004 Jeep, operated by Bruce H. Vanbrink of Cooperstown, which then exited the shoulder of the roadway and overturned onto its roof.

The operator of the Dodge van, Jonathan A. Neitzey, 23, from Bowie, Md., was transported to Bassett Medical Center in Cooperstown with minor injuries.

Theresa Woolsey, 46, of Brookfield, was pronounced dead at the scene; Douglas Woolsey, 50, of Brookfield, was transported to Bassett Medical Center in Cooperstown where he was pronounced deceased.

Bruce H. Vanbrink, 56, of Cooperstown, was transported to Bassett Medical Center with non-life threatening injuries.

The investigation continues.
