The Chronic Disease Self-Management Program is a six week course that meets for 2 and 1/2 hours, once a week. CDSMP was developed at Stanford University Patient Education Research Center as an evidence-based workshop that recognizes the underlying issues that affect all people with chronic conditions.
Chronic disease includes conditions such as heart disease, diabetes, arthritis, neuropathy, asthma, high blood pressure, stroke, anxiety and depression to name a few. Leaders will cover topics such as problem solving, active living with chronic disease, decision making, pain and fatigue management, better breathing, and dealing with difficult emotions.
The workshop is open to all people with chronic diseases or caregivers of people with chronic diseases. The program has been shown to help participants increase quality of life, increase exercise, decrease symptoms such as depression and shortness of breath, and utilize emergency healthcare less.
There will be an information session at the Office for the Aging, 138 Dominic Bruno Blvd., Canastota Sept. 20 th at 10am. The Rural Health Council of Madison County will be starting classes at the Office for the Aging, in early October. There is no fee for the class except a $20 deposit for the book that is returned at the end of the 6 weeks if the participant chooses to return the book.
